Features pediatric symbols in proportionally-spaced lines (ETDRS format) for monocular vision testing. Line sizes range from 10/100 to 10/4 (3/30 to 3/1.2) equivalent. The 10/80 (3/24) and 10/50 (3/15) lines have been omitted. Has a hole for hanging the chart on the wall. Includes response key and instructions.

Notations on the left side of the chart include actual letter size for 10 foot and 3 meter measurements. Equivalent letter size notations found on the right side of the chart include 20 foot, 6 meter, decimal, and log MAR measurements.
